Engine failure

Following from Brian Watkinson's item about water in the oil, I thought it might be useful to ask who has suffered an engine failure and what the cause might have been.

Here's a few from me:

MLL721 (1972) no 2 big end failure;
RM2156 (2002) loss of oil pressure caused by oil way end cap bolt failing
RM1394 (2002) engine failure, cause not known
RFW14 (2010) no.2 big end failure
RF136 (1994) engine knocking, start of big end failure at low speed, caught it in time!
RM1571 (1998) Leyland engine failed, overheating, replaced with AEC unit.
GS67 (1975) broken crankshaft after rebuild.

Has only happened twice.....

RM1842 av590 was worn out then stopped with A bang.
Av 690 replacement for above, oil pick up pipe from sump incorrectly fitted.
Managed 500 miles before seizing on way to RM 50....
